Ever concerned that missing breakfast could leave you mentally hazy at work? Or that intermittent fasting might make you irritable, unfocused and less productive?
Snack-food adverts tell us that "you're not you when you're hungry", reinforcing the widespread idea that eating is necessary to keep our minds sharp.
This belief is firmly embedded in our culture. We are often told that continual refuelling is the key to remaining alert and productive.
However, time-restricted eating and intermittent fasting have become enormously popular wellbeing approaches over the past decade. Millions of people follow them for potential long-term gains, ranging from weight management to better metabolic health.
That poses an important question: is it possible to gain the health benefits of fasting without losing our cognitive edge? To investigate, we carried out the most wide-ranging review so far of fasting's effects on cognitive performance.
Why fast in the first place?
Fasting is more than a fashionable dietary shortcut. It draws on a biological mechanism shaped over thousands of years to help humans endure periods of scarcity.
When we eat at regular intervals, the brain relies chiefly on glucose, which the body stores as glycogen. After roughly 12 hours without eating, however, those glycogen reserves begin to fall.
The body then makes an ingenious metabolic change: it starts converting fat into ketone bodies, such as acetoacetate and beta-hydroxybutyrate, that can serve as an alternative source of fuel.
This metabolic adaptability, once vital to our ancestors' survival, is now associated with a range of potential health advantages.
Some of fasting's most encouraging effects arise from the way it alters internal bodily processes. For example, fasting triggers autophagy, a cellular "cleanup crew" that removes damaged material and recycles it - a process believed to promote healthier ageing.
It can also increase insulin sensitivity, helping the body regulate blood sugar more efficiently and reducing the risk of conditions including type 2 diabetes.
In addition, the metabolic changes brought about by fasting may provide wider protection by lowering the chance of chronic diseases commonly linked with overeating.
What the data showed about fasting and cognitive performance
These physical benefits have helped make fasting appealing. Yet many people are reluctant to try it because they fear that their mental performance will collapse without a regular intake of food.
To examine this concern, we conducted a meta-analysis - a "study of studies" - of all available experimental research comparing people's cognitive performance while fasting with their performance after being fed.
Our search found 63 scientific articles, covering 71 independent studies, involving a total of 3,484 participants who completed 222 different measures of cognition. The studies covered almost seven decades, from 1958 to 2025.
Once we combined the findings, the result was straightforward: among healthy adults, there was no meaningful difference in cognitive performance between fasted and satiated states.
Participants did equally well in tests of attention, memory and executive function, regardless of whether they had eaten recently.
When fasting does matter
Our analysis did, however, identify three important factors that may alter fasting's effects on the mind.
First, age matters. Fasting produced no measurable reduction in adults' mental performance. Children and adolescents, by contrast, performed less well in tests when they missed meals.
Their developing brains appear to be more vulnerable to changes in energy availability. This supports the long-established recommendation that children should have a proper breakfast before school to aid learning.
The timing of the fast also appears relevant. Longer fasts were linked with a narrower performance difference between fasted and fed states. This may reflect the metabolic transition to ketones, which can provide the brain with a consistent energy supply as glucose becomes depleted.
Fasted participants generally performed less well when they were tested later in the day, indicating that fasting may intensify the natural lows in our circadian rhythms.
The kind of test was important, too. In cognitive tasks using neutral symbols or shapes, fasting participants performed equally well and occasionally even a little better.
Yet when tasks featured food-related cues, fasted participants faltered. Hunger does not cause universal brain fog, but it can make us more distractible when food is at the forefront of our thoughts.
What this means for you
For most healthy adults, these findings should be reassuring: you can try intermittent fasting or other fasting protocols without fearing that your mental sharpness will disappear.
Even so, fasting is not suitable for everyone. Children and teenagers warrant particular caution, as their brains are still developing and they seem to require regular meals to perform at their best.
Likewise, fasting may be more difficult to maintain if your work demands maximum alertness late in the day or if you regularly encounter tempting food cues.
Naturally, some groups - including people with medical conditions or particular dietary requirements - may be ill-advised to fast without professional advice.
Ultimately, fasting is better regarded as an individual tool than a universal recommendation. Its advantages and difficulties will vary from one person to another.
