Why You Wake Between 2 am and 4 am: Cortisol as the Night-Time Alarm Clock

Often, an inconspicuous hormone is responsible.

Many people regularly wake abruptly between 2 am and 4 am, wondering what is wrong with their body. Rather than sleeping soundly through the night, their thoughts begin to race, their pulse quickens and getting back to sleep feels almost impossible. This familiar time window is not random: it is caused by a complex interaction between the body clock, the stress response and energy metabolism, with one particular hormone playing the leading role.

Why specifically between 2 am and 4 am?

The human body operates according to a distinct day-night pattern known as the circadian rhythm. During the second half of the night, the brain shifts from deep, slow-wave sleep into lighter sleep stages and REM sleep, when dreaming is particularly intense.

At this point, the body is considerably more sensitive to disruption. Noise, internal restlessness or fluctuations in blood sugar can therefore make it easier to wake up - and remain awake.

Between 2 am and 4 am, the body is in a biological “vulnerability zone”, where even minor imbalances have a greater impact.

At the same time, the body has to cope with the overnight fast: there has been no food for hours, yet the brain still requires a steady supply of energy. If this balancing act goes wrong, the body switches on its internal alarm system and ends sleep in order to stabilise metabolism.

Cortisol: the stress hormone acting as a night-time alarm clock

At the centre is a hormone that many people primarily associate with burnout or pressure at work: cortisol. Produced by the adrenal glands, it is a key part of the stress response.

In normal circumstances, cortisol levels are low overnight. From around 2 am, they begin to rise gradually so that we are awake and able to function in the morning. This gentle increase is part of the body's usual daily rhythm.

What happens during prolonged stress

When someone experiences ongoing stress over a prolonged period - at work, privately or internally - this rhythm can become disrupted. The rise in cortisol may then:

  • begin too early;
  • be too steep; or
  • occur in waves.

As a result, the body effectively starts its “morning programme” in the middle of the night. This can show up as symptoms such as:

  • suddenly waking up for no obvious reason;
  • racing thoughts, overthinking, planning or worrying;
  • a noticeable pulse or heart palpitations;
  • a dry mouth or mild sweating.

The body behaves as though an important appointment is due - except it is 3 am in the dark.

When blood sugar drops too low overnight

Alongside cortisol, sugar metabolism is another important factor. After the evening meal, the body has to rely on its reserves for many hours. Cells obtain their energy from stores in the liver and muscles.

If the final meal of the day is poorly composed, low blood sugar can develop during the night. Two extremes are particularly risky:

  • very light, low-carbohydrate evening meals that do not sustain the body for long;
  • meals high in rapidly absorbed sugars, such as sweets, white bread, fizzy drinks or rich desserts.

After a sugar spike, blood sugar often drops noticeably. If it falls too low during the night, the brain interprets this as an alarm, because it can function only to a limited extent without sufficient sugar.

In response, the body releases adrenaline and cortisol to mobilise energy again. While this protects the system, it also brings sleep to an end. Typical signs include:

  • suddenly waking with heart palpitations;
